Repeat Prescriptions - Preferred Chemist
IMPORTANT PATIENT INFORMATION
Our computer system now specifies which is your preferred pharmacy and this is printed in the top left corner of your prescription (a blank box indicates your choice to collect prescriptions from the Cullompton surgery). To reduce the risk of confusion, we will, with effect from 1st January 2017, be phasing out the tick-boxes on repeat slips, and all your prescriptions will be prepared in accordance with the choice you have made which is recorded on our computer system. Please check the top left corner of your prescription to ensure that this correctly reflects your choice. If there are any changes to be made please notify us in writing by 1st January 2017. Should your preferred collection point change after this date, notification needs to be made in writing for your computer record to be updated.
Patients on regular medication will require repeat prescriptions. Each time a repeat prescription is written, a new request slip is produced for you to make your next request for regular medication.
To send a request you can:
- Post in the Request: please use your normal repeat slip and send to the surgery. You need to enclose SAE if you wish to have it posted back.
- Come To The Surgery: Once the slip has been filled in you can come into the surgery reception and hand it in.
- Online: You can book your repeat prescriptions via our Online Services.
Once the request has been received, you must allow 48 Hours or Two Working Days (excluding Saturday, Sunday and Bank Holidays) before you will be asked to collect your prescription.
Please read the repeat slip carefully. We may sometimes put special information on it for you.
We operate a dispensing service for patients registered at Bradninch, Uffculme and Sampford Peverell.
We are sorry but we cannot accept telephone prescription requests.
Filling in Your Repeat Prescription Slip
When filling in your Repeat Prescription Slip, you must make sure the following are clearly marked:
- Medication Required
- Strength Required
- Quantity Required
If you urgently need medication, contact your prescriber immediately to arrange a prescription.
If this isn't possible, you may be able to get medicine from a pharmacist in an emergency, subject to certain conditions.
